Responsibilities

• Conduct home visit Physical Therapy assessments to evaluate patients' medical conditions and rehabilitation potential.
• Provide direct patient care to enhance functional capabilities and limitations.
• Utilize EMR documentation efficiently.

Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Proficient in EMR documentation.
• CPR Certification.
