Job Description

Paradigm Rehab is seeking a dedicated Rehabilitation Therapist to help individuals reconnect with their environment and enhance their quality of life. As a valued member of our team, you will conduct home visit assessments, provide direct patient care, and contribute to the rehabilitation process.

Responsibilities

• Evaluate medical conditions and functional capabilities of referred patients.
• Deliver compassionate and effective rehabilitation services.
• Document patient progress utilizing EMR systems.
• Maintain an unencumbered Physical Therapy license for New York.

Requirements

• Graduate from an accredited Physical Therapy program.
• Valid CPR certification.
• Proficient in EMR documentation.
• Ability to conduct in-person assessments in Brooklyn.
