Requirements

• Graduate from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Perform home visit assessments, offering direct patient care to evaluate medical conditions and potential for rehabilitation.
• Proficient in EMR documentation.
• Current CPR Certification.
