Job Description
We are looking for a dedicated Registered Nurse (RN) Field Nurse for Home Care to join our team at ChristianaCare. This position involves providing skilled nursing care and treatment to geriatric patients in the community, developing care plans, and ensuring patient-centered care.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Develop and implement goal-oriented, patient-centered care plans.
- Ensure IV proficiency and perform IV functions.
- Develop discharge plans for patients.
Requirements
- BSN required; ADN candidates must obtain BSN within three years of employment.
- Current DE RN license or compact state license.
- 2 years of HomeHealth RN experience within the last 12 months.
- Previous Home Healthcare and Med-Surgical experience preferred.
- Willingness to train and perform IV functions.
