We are looking for a dedicated Registered Nurse (RN) Field Nurse specializing in Home Care to join our team at ChristianaCare. As part of our HomeHealth department, you will provide skilled nursing care to geriatric patients in the community, ensuring top-notch treatment and support. Responsibilities

• Provide skilled nursing care and treatment to geriatric patients in the community.
• Develop and implement patient-centered care plans in line with available resources.
• Assist in developing discharge plans for patients.

Requirements

• BSN required; ADN candidates must obtain BSN within three years of employment.
• Current DE RN license or compact state license.
• 2 years of recent HomeHealth RN experience.
• Previous Home Healthcare and Med-Surgical experience preferred.
