Essential Job Functions:

• Deliver and coordinate quality patient care in compliance with physician orders.
• Assess patient condition, report changes, and update care plans accordingly.
• Document all activities and maintain accurate patient records.
• Ensure infection control and educate patients on self-care techniques.

Requirements:

• Graduate of an accredited nursing school.
• Current, unrestricted LPN license in the state of practice.
• CPR certification and proficiency in clinical assessments.

Preferences:

• Six months clinical care experience as an LPN.
• Familiarity with home health care.

Skills/Abilities:

• Attention to detail, time management, and effective problem-solving skills.
• Strong communication and organizational abilities.

Physical Requirements:

• Ability to lift 50 pounds and assist patients as needed.
• Must have physical and mental capacity to respond to emergencies.
• Function in various environments possibly exposed to allergens.
