Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit assessments to evaluate patient conditions and potential for rehabilitation.
• Experience with EMR documentation preferred.
• Current CPR Certification.
