Responsibilities

• Conduct home visit assessments of referred patients to evaluate medical conditions and rehabilitation potential.
• Provide direct patient care and implement therapeutic interventions to improve functional capabilities.
• Utilize EMR documentation for accurate record-keeping and care coordination.
• Maintain CPR certification and adhere to professional standards and regulations.

Requirements

• Graduate from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license in New York.
• Proficiency in EMR systems (experience with Hello Note is a plus).
• Flexible scheduling options available for full-time, part-time, and per diem positions.
