Paradigm Rehab is seeking a dedicated and compassionate Physical Therapy Specialist to join our team. As a Physical Therapist, you will have the opportunity to work closely with patients, assessing their conditions, and providing direct care to enhance their quality of life.

Responsibilities

• Conduct home visit assessments to evaluate patients' medical conditions and functional capabilities.
• Utilize EMR documentation for accurate recording of patient information.
• Collaborate with patients to create effective rehabilitation plans.

Requirements

• Graduate from an accredited Physical Therapy program.
• Hold a valid Physical Therapy license for the state of New York.
• Proficient in conducting physical assessments and creating treatment plans.
• Current CPR certification.
