Physical Therapist Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Perform home visit Physical Therapy assessments, providing direct patient care and evaluating medical conditions.
• Proficiency in EMR documentation (experience with Hello Note is a plus).
• CPR Certification.
