Responsibilities

• Conduct home visit assessments of patients to evaluate medical conditions and rehabilitation potential.
• Provide direct patient care focusing on improving functional capabilities.
• Document patient information accurately using EMR systems.

Requirements

• Graduation from an accredited Physical Therapy program.
• Active Physical Therapy license for New York.
• Proficiency in EMR documentation.
• CPR Certification.
