Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct thorough home visit assessments to evaluate medical conditions and rehabilitation potential.
• Proficient in EMR documentation, experience with Hello Note is a plus.
• Hold current CPR Certification.
