Nurse-LPN MDS Coordinator

Job Description

We are looking for a dedicated Nurse-LPN MDS Coordinator to join Genesis Healthcare, a leading provider of healthcare services. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing key items on the Minimum Data Set (MDS) to ensure proper patient care and nursing center reimbursement.

Position Highlights

• Direct the interdisciplinary RAI process and ensure accurate completion, encoding, and electronic transmission of the MDS.
• Contribute to developing individualized resident Care Plans and identifying resident needs and outcomes per regulations.
• Train new team members on clinical content and electronic completion of MDS items.

Requirements

• Must be a licensed LPN in good standing.
• Prior experience in a healthcare facility is required with knowledge of nursing practices and regulations.
• Proficiency in windows applications is necessary; AccuMed experience is a plus.
• Previous experience as an MDS Coordinator and familiarity with RAI/PPS processes is preferred.
• Current BLS/CPR certificate required.
