Minimal Data Set Coordinator - LPN

Job Description

We are looking for a dedicated Minimal Data Set Coordinator with an LPN certification to join Genesis HealthCare. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and related Care Area Assessments (CAAs). Your work will ensure proper patient care and accurate nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process, ensuring compliance with guidelines for completion, encoding, and State transmission of the MDS.
• Contribute to the development of personalized resident Care Plans, identifying resident needs and projected outcomes in accordance with Federal and State regulations.
• Provide training to new team members on clinical content and electronic completion of relevant MDS items.

Requirements

• Active LPN license in good standing.
• Prior experience in hospital, long-term care, or related healthcare settings.
• Familiarity with general, rehabilitative, and restorative nursing practices and healthcare regulations.
• Proficiency in Windows applications; AccuMed experience preferred.
• Previous experience as an MDS Coordinator and demonstrated proficiency in RAI/PPS processes.
• Current BLS/CPR certification.
