Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

Genesis Health, a leading healthcare services provider, is seeking a skilled Minimal Data Set Coordinator who is a licensed practical nurse. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing necessary items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Position Highlights

• Direct the interdisciplinary RAI process and ensure compliance with MDS guidelines.
• Develop individualized resident Care Plans in adherence to Federal and State regulations.
• Provide training on clinical content and electronic completion of MDS items to new team members.

Requirements

• Current LPN license in good standing.
• Prior experience in a healthcare facility is required.
• Familiarity with nursing practices, medical procedures, and long-term care regulations.
• Proficiency in Windows applications and AccuMed is preferred.
