Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

We are looking for a dedicated Minimal Data Set Coordinator who is a Licensed Practical Nurse to join Genesis Health's team. The LPN MDS Coordinator will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and proper nursing center reimbursement.

Position Highlights

• Lead the nursing center interdisciplinary Resident Assessment Instrument (RAI) process in compliance with guidelines and regulations.
• Contribute to the development of individualized resident Care Plans, addressing resident needs and expected outcomes.
• Provide training to new interdisciplinary team members on clinical content and electronic MDS completion.

Requirements

• Active LPN license in good standing.
• Prior experience in hospital, long-term care, or related healthcare facilities.
• Familiarity with nursing practices, medical procedures, and relevant regulations.
• Proficiency in windows applications with AccuMed experience preferred.
• Previous MDS Coordinator experience is a plus.
• Current BLS/CPR certification required.
