Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

Genesis Health is seeking a qualified Licensed Practical Nurse to join as a Minimal Data Set Coordinator. As an LPN MDS Coordinator, you will play a vital role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure appropriate patient care and nursing center reimbursement.

Position Highlights

• Direct the interdisciplinary Resident Assessment Instrument (RAI) process.
• Develop individualized resident Care Plans.
• Provide training on clinical content and electronic completion of MDS items.

Requirements

• LPN in good standing and currently licensed by the state.
• Previous experience in healthcare facilities.
• Knowledgeable of nursing practices and procedures.
• Computer skills including windows applications.
• AccuMed experience preferred.
• Prior experience as an MDS Coordinator preferred.
• Demonstrated proficiency in RAI/PPS processes.
• Current BLS/CPR certificate required.
