Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

We are looking for a skilled Minimal Data Set Coordinator who is a Licensed Practical Nurse to join the Genesis Health team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure high-quality patient care and proper nursing center reimbursement.

Position Highlights

• Direct the interdisciplinary Resident Assessment Instrument (RAI) process.
• Develop individualized resident Care Plans following Federal and State regulations.
• Train new team members on clinical content and electronic MDS completion.

Requirements

• LPN in good standing with current state license.
• Prior experience in a healthcare-related facility.
• Proficiency in general, rehabilitative, and restorative nursing practices.
• Computer skills with knowledge of Windows applications.
• AccuMed experience preferred.
• Previous MDS Coordinator experience is a plus.
• BLS/CPR certificate required.
