Job Description

Genesis HealthCare, a prominent healthcare services provider, is currently seeking a dedicated LPN for the position of LPN MDS Coordinator. As part of our team, you will play a crucial role in the Resident Assessment Process by completing important sections of the Minimum Data Set (MDS) and related Care Area Assessments (CAAs), ensuring high-quality care and proper reimbursement for services provided.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process in compliance with standards, including MDS completion, encoding, and electronic transmission.
• Contribute to the development of personalized resident Care Plans according to Federal and State requirements.
• Provide training on clinical content and electronic MDS completion to new team members.

Requirements

• Active LPN license with relevant experience in healthcare settings.
• Familiarity with general nursing practices, medical procedures, and long-term care regulations.
• Proficiency in Windows applications and AccuMed software is preferred.
• Prior experience as an MDS Coordinator with knowledge of RAI/PPS processes.
• Current BLS/CPR certification.
