Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our ChristianaCare HomeHealth team. As an LPN, you will provide skilled nursing care to geriatric patients, implement care plans, and participate in discharge planning. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented, patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
