Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) specializing in Home Health Care to join our team at ChristianaCare. As part of our home health division, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and contribute to discharge planning, all while ensuring patient-centered and goal-oriented care.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement patient-centered and goal-oriented care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
