Job Description
ChristianaCare HomeHealth is seeking experienced Licensed Practical Nurses (LPNs) to join our team in New Castle County, DE. As part of our team, you will provide skilled nursing care to geriatric patients in the community, implement patient-centered care plans, and contribute to discharge planning.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Must possess a current DE LPN license or compact state license.
