Job Description
ChristianaCare HomeHealth is seeking Licensed Practical Nurses (LPNs) to join our team. As an LPN in our Home Care division, you will provide skilled nursing care and treatment to geriatric patients in the community. Working with a supportive leadership team, you will play a crucial role in implementing care plans, participating in discharge planning, and ensuring patient-centered care.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
