Job Description

Genesis Health, a leading provider of healthcare services, is seeking a Licensed Practical Nurse (LPN) to join our team. As an LPN MDS Coordinator, you will play a vital role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Provide training on clinical content and electronic completion of MDS items to new team members.

Requirements

• Active LPN license.
• Prior experience in a healthcare facility.
• Knowledge of nursing practices, regulations, and procedures.
• Computer skills, including proficiency in Windows applications.
• AccuMed experience preferred.
• Previous experience as an MDS Coordinator is a plus.
• BLS/CPR certification required.
