Join Genesis HealthCare, a leading provider of healthcare services, as a Licensed Practical Nurse (LPN). As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs). Your responsibilities include directing the interdisciplinary RAI process, developing individualized Care Plans, and training team members on clinical content and electronic MDS completion.

Position Highlights

• Direct the RAI process and ensure accurate completion, encoding, and transmission of MDS.
• Develop individualized Care Plans and oversee outcome projections.
• Train team members on clinical content and electronic MDS completion.

Requirements

• Active LPN license.
• Prior experience in a healthcare facility.
• Knowledge of nursing practices and regulations.
• Computer skills, including Windows applications.
• AccuMed experience preferred.
• Proficiency in RAI/PPS processes and MDS completion.
• Current BLS/CPR certificate.
