We are seeking dedicated Home Care LPNs to join our team at ChristianaCare. As a HomeHealth LPN, you will provide skilled nursing care to geriatric patients in the community, implementing care plans and ensuring patient well-being.

Responsibilities

• Provide skilled nursing care and treatment to geriatric patients.
• Implement patient-centered care plans in alignment with available resources.
• Participate in developing discharge plans for patients.

Requirements

• High School Diploma or GED required.
• Current DE LPN license or compact state license.
