Job Description
We are currently looking for a dedicated and experienced Home Care Licensed Practical Nurse (LPN) to join ChristianaCare. As part of our HomeHealth team, you will have the opportunity to provide skilled nursing care to geriatric patients in the community while working with a supportive leadership team committed to your success.

Responsibilities
- Deliver nursing care and treatments to patients following care plans.
- Participate in discharge planning for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Active DE LPN license or compact state license.
Job Details
- Job Types: Per diem, Part-time, Full-time
- Pay: $22.00 - $28.00 per hour
- Expected hours: 20 – 40 per week
- Weekly schedule: Monday to Friday, Rotating weekends
- Work Location: In person
