As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure appropriate patient care and reimbursement.

Position Highlights

• Direct the RAI process and ensure compliance with guidelines for MDS completion.
• Develop individualized resident Care Plans in accordance with Federal and State regulations.
• Provide training on clinical content and electronic completion of MDS items.

Requirements

• LPN in good standing with current state license.
• Prior experience in hospital or long-term healthcare facility.
• Proficiency in general, rehabilitative, and restorative nursing practices.
• Computer skills, including Windows applications.
• AccuMed experience preferred.
